چکیده

INTRODUCTION Most plantar foot ulcers in diabetic patients are caused by a combination of neuropathy and elevated plantar pressure and develop in the regions of forefoot and toes. Casting devices (i.e. Total Contact Cast or Mabal shoe) are commonly used for offloading these ulcers. Alternatively, in many diabetic foot centers where casting technicians are absent, prefabricated forefoot offloading shoes (FOS) are used for ulcer treatment. However, little is known about the biomechanical effectiveness of the FOS. Therefore, the purpose of this study was to assess the effectiveness of FOS in offloading the forefoot in diabetic patients with peripheral neuropathy.

Surgical offloading procedures for diabetic foot ulcers compared to best non-surgical treatment: a study protocol for a randomized controlled trial

Background Diabetic foot ulcers are frequently related to elevated pressure under a bony prominence. Conservative treatment includes offloading with orthopaedic shoes and custom made orthotics or plaster casts. While casting in plaster is usually effective in achieving primary closure of foot ulcers, recurrence rates are high. Minimally invasive surgical offloading that includes correction of f...

Pedobarographic and kinematic analysis in the functional evaluation of two post-operative forefoot offloading shoes

BACKGROUND Forefoot offloading shoes are special orthopaedic footwear designed to protect and unload the injured part of the foot after surgery and for conservative treatments. The offloading action is often achieved by transferring plantar load to the rearfoot via rocker shoes with reduced contact area between shoe and ground. While these shoes are intended to be worn only for short periods, a...

Use of Pressure Offloading Devices in Diabetic Foot Ulcers

OBJECTIVE Pressure mitigation is crucial for the healing of plantar diabetic foot ulcers. We therefore discuss characteristics and considerations associated with the use of offloading devices. RESEARCH DESIGN AND METHODS A diabetic foot ulcer management survey was sent to foot clinics in all 50 states and the District of Columbia in 2005. A total of 901 geographically diverse centers responde...

Offloading Foot Wounds in People with Diabetes

Vol. 26, No. 1 January 2014 13 Abstract: Up to 25% of people with diabetes will develop a foot ulcer at some point during their lifetime, and 1 in 5 will require an amputation. In 2006, more than 65,700 lower-limb amputations were performed in the United States alone in people with diabetes, and the 5-year mortality rate following an amputation is worse than for most malignancies. Along with an...

Footwear and offloading interventions to prevent and heal diabetic foot ulcers: Adapted IWGDF guidance

Introduction: Foot ulcer is one of the greatest complications of diabetes. If there is no effective pressure reduction in the ulcer, healing may be delayed. In this regard, the use of offloading methods for the prevention and treatment of foot ulcers in diabetic patients is known as a common therapeutic approach. Since evidence-based recommendations should be considered for the use of the above...
